iPad Paperwork Packet (Included in Handbook)

Dear Parents or Guardians,

Please be sure to read these instructions carefully, as incomplete or incorrect paperwork will be returned to you and result in a delay for your child receiving his or her iPad.

NOTE: If your family has applied and qualified for the free and reduced lunch program, please contact Lois Straber at 516.277.7815 or straberl@northshoreschools.org prior to filling out any paperwork for instructions on how to qualify for a protection plan fee waiver.


To receive a district iPad, a parent/guardian and student must do the following:

1. Click the first link at the top of this page titled "iPad Handbook" to open the iPad Handbook and review the contents carefully

2. Click the second link titled "iPad Paperwork Packet" and print all pages

3. The parent and child must sign and date all forms

4. All forms and payments must be returned in-person or by mail only to:
